Practice Nurse – Clinic 1 FTE

This position is fulltime, Monday to Friday and is based at our Rawene location.

We are seeking a Registered Practice Nurse to help keep our diverse range of patients well informed, engaged and supported. Ideally you will have experience working with vulnerable and chronic care patients. You will have a high standard of clinical skills and will be given the opportunity to develop and utilise your knowledge and leadership skills with the support of our GPs. Your experience and skills will include:

• Current New Zealand Practicing Certificate and Indemnity Insurance
• Competent in areas of nursing in primary care
• Willing to continually learn and improve
• Excellent communication and interpersonal skills
• Ability to build strong rapport with team members
• You will ideally hold current CPR certificate
• Immunisation and smear taker certificates are desirable but not essential
• NZ full New Zealand driver’s license

Community Support Worker – 1 FTE


Hokianga Health Enterprise Trust

The role of the Community Support Worker is to provide guidance and support at the level necessary for clients to have a safe and satisfying life. These services and support are provided to individuals and/or their whanau to ensure positive responses to changing aspirations, capacity/capability, goals, and mental health challenges over time.

Effective communication is vital for this role as you will be working alongside clinical teams and other community agencies to ensure a wrap around service to the people, we support. Your experience and skills will include:

• National Certificate or Diploma in Mental Health support or an equivalent relevant qualification.
• Excellent organisational and time management skills, are key to this role
• Experience to know when and how to engage with whanau
• Experience/understanding of a client centered, recovery-focused approach
• Well developed written and verbal communication, networking, and administrative skills, including data entry and case management
• A full New Zealand driver’s license
